The Hidden Third: When Pelvic Pain Is Actually Venous

Up to 30% of chronic pelvic pain may be venous in origin — here's how to recognize it, differentiate it, and refer. Some of the most frustrating pelvic-pain cases aren't treatment-resistant — they're mislabeled. Research suggests that as much as a third of chronic pelvic pain in women may be venous in origin: reflux or obstruction in the veins draining the pelvis, a condition called pelvic venous disorder (PeVD).

Some of the most frustrating pelvic-pain cases aren't treatment-resistant — they're mislabeled. Research suggests that as much as a third of chronic pelvic pain in women may be venous in origin: reflux or obstruction in the veins draining the pelvis, a condition called pelvic venous disorder (PeVD). It's common, it's treatable, and it's routinely missed because it mimics the myofascial pain we're trained to treat.

The pattern that gives it away. Venous-origin chronic pelvic pain has a signature. It's a dull ache present throughout the menstrual cycle, notably pain-free in the morning, and building through the day with standing, walking, and load. It's relieved by lying down and by heat. The single most useful historical detail is prolonged post-coital aching — the kind that lingers well past the encounter. Add menorrhagia, deep dyspareunia, and no relief from NSAIDs, and the picture sharpens....

The exam discriminator. This is where your hands do the diagnostic work. Venous-origin pain presents with diffuse uterine and surrounding tenderness — but crucially, without the pelvic-floor muscular tenderness that reproduces pain in myofascial cases. You may see abnormal veins on the vulva, thigh, or abdominal wall. If pelvic-floor palpation reproduces the pain, think myofascial. If tenderness is diffuse and the floor is quiet, think venous. If it also feels a little "puffy" inside... venous should be considered

Why this changes your hands. Here's the clinical stakes: aggressive pelvic-floor strengthening can worsen venous symptoms. So reading the pattern first isn't academic — it protects your patient from a well-intentioned plan that backfires. For suspected PeVD, favor normalizing tone, relaxation, posture, and compression education over loading. Focus on lymphatic support, abdominal muscle coordination and tone without gripping. Teach to unclench the glutes and thighs. Teach postural changes and rest positions like legs up a wall.

When to refer. If pain persists two to three months despite conservative care, refer for transvaginal ultrasound — an accessible screen where arcuate veins over 5mm crossing the uterus with reversed flow on Valsalva are highly predictive. From there, interventional radiology can confirm with venography and offer embolization, which shows meaningful, durable pain reduction. Route flank pain and hematuria to urology, and differential questions to gynecology.

One more connection worth knowing: there's a high overlap of PeVD with hypermobile EDS and POTS, and embolization can improve autonomic symptoms too. Screen for connective-tissue features when you assess pelvic pain.

You may be the first clinician to name this for your patient. That's not a small thing — it's the difference between years of dismissal and a treatment path that finally fits.
